functional methods and volume of researches of lungs at children with a mucoviscidosis

The estimation of ventilating function of a respiratory tract was spent by spirometry methods (54 children at the age of 4-18 years) on device MasterScreenIOS firms E.Jaeger (Germany) with the special adaptation for carrying out of an impulsive oscillometry.

The method of an impulsive oscillometry (IOS) surveys 56 children at the age from 3 years of 7 months.

Bronhofonografija (BFG) with use of programs Pattern, Pattern Analizer, Work it has been executed at 60 children observed by us at the age from 2 months till 10 years.

Bronhofonografija - the method of an estimation of a respiratory pattern - is based on the analysis of peak-frequency characteristics of a spectrum of respiratory hums [70]. Registration of respiratory hums is carried out by means of the gauge possessing high sensitivity in a wide strip of frequencies, including frequencies which are not taped at auscultation and have the important diagnostic value. The set of the special filters intended for formation of a frequency spectrum which contains the information on the specific acoustic phenomena arising during a respiratory cycle enters into the complex hardware also. Scanning of a pattern of breath is made in a frequency range from 0,1 to 12,6 kHz. The graphic display received on the monitor fonorespirogrammy has been named «a breath pattern» [70].

Basis of method BFG is the analysis of time and frequency characteristics of a spectrum of the respiratory hums registered during a respiratory cycle. Received in the three-dimensional image fonorespirogrammy (respiratory hums) are presented to the acoustic
Ranges: АРД0 a base range of 100-1200 Hz, t.e a range of normal breath, ARDz a low-frequency range of 1201-5000 Hz; АРД2 high-pitched 5000-12600 Hz and АРДі the general range of 1200-12600 Hz. Results were estimated by means of applied programs [70, 97, 99]. Besides an acoustic equivalent of work of the respiratory muscles, expressed in ndzh, in the specified ranges, used factor (), reflecting the same parametres in relative units: К-і - reflects all spectrum of frequencies; К2 - a high-pitched range and Kz - a low-frequency range.

Procedure of registration of a pattern of respiratory hums is made in position sitting, at quiet breath, during a short time interval (4-10 seconds). The gauge microphone is mounted in a mouthpiece and is located in an oral cavity, the sensitive element of the gauge is referred towards a larynx.

At children of younger age the mask which fastens to a microphone is used. The received acoustic signal is transformed to the discrete form, processed by the processor. Received on the monitor screen survey fonorespirogrammy in the three-dimensional image are parted on an axis of frequencies by the cursor which is border of diagnostic zones [70, 97, 99]. At carrying out BFG the special nasal clamp is used, carried out five consecutive repeated respiratory maneuvers. Research carried out twice at entering in a hospital and after therapy.

Considering, that an estimation of passableness of respiratory tracts on the basis of the analysis the curve stream-volume demands performance of the maximum and forced maneuvers of the exhalation, the given kind of research, in the standard opinion, children aged can carry out is more senior 5 years. The spirometry was spent by a standard technique to children from 4 summer
Age, the method of a pneumotachometry with registration curve "stream-volume" of the forced exhalation on device MasterScreenIOS was used.

According to the traditional plan of inspection to all children with MV paraclinic methods of researches were spent also: the developed general analysis of blood; the biochemical analysis of blood and proteinogramma; blood serum immunoglobulins; the general analysis of urine; a scatology; sputum crops on flora and sensitivity to antibiotics; roentgenography of organs of a thorax (in two projections) and adnexal sinuses of a nose; a multispiral computer tomography of organs of a thoracal cavity and adnexal sinuses of a nose (under strict indications); an electrocardiogram; the ECHO-KG with pressure definition in a pulmonary artery; ultrasonic of organs of an abdominal cavity with doplerografiej vessels of a liver and adnexal sinuses of a nose; research of function of external breath; potovyj the test; DNA diagnostics; feces research on pancreatic elastazu-1.

Special methods of research have been carried out: definition of indicators of function of external breath by a method a curve stream-volume, a method of an impulsive oscillometry, bronhofonografii,

bodipletizmografii; research of gas structure arterializirovannoj bloods; rinomanometricheskoe research; monitoring pulse rates (CARDIAC CONTRACTIONS RATE), frequencies of respiratory movements (CHDD), pikfloumetrija and measurement of saturation of haemoglobin by oxygen.

The inspection complex was spent twice: at entering of the patient in a hospital and before an extract. Monitoring of pulse, frequency of breath, saturation of haemoglobin by oxygen and pikfloumetrija were spent daily. The volume and methods of the spent researches are resulted in table 2.

Methods of statistical processing

Statistical processing of the received results was spent with use of the personal computer by methods of the mathematical
Statisticans by means of the program «Statistica 6 for Windows». Differences between groups considered statistically significant at probability of the forecast (r
