Fig. № 1. Structure and functions of the Republican centre of the mediko-social help to children and teenagers with chronic hepatitises

Dispensary observation over children and teenagers with HG

Prophylactic medical examination - active dynamic observation over a state of health of children and teenagers with HG, including a complex of preventive, diagnostic and medical-improving actions.

The dispensary observation for children with HG is carried out for the purpose of the control over a disease current, selection carrying out on PVT, inspection before appointment PVT, observation over a condition of the patient during PVT, an estimation of efficiency PVT, observation over patients after carrying out PVT.

The patients who are on the dispensary account, are sectioned into 5 groups:

1. Patients who pass inspection for the decision of a question on carrying out PVT

2. The patients receiving PVT

3. The patients who have finished PVT

4. Patients to whom it is contraindicative PVT

5. Children born from mothers with NV by a -infection and NANOSECOND by a -infection.

1 dispensary group

Defining necessity and possibility of appointment PVT it is necessary to understand, that the purpose of antiviral therapy is eradikatsija a virus of a hepatitis and prevention of development of complications, assotsiirovannyh with a hepatitis, including a necrotic inflammation, a fibrosis, a cirrhosis, a hepatocellular carcinoma and a lethal outcome. A final point of antiviral treatment is achievement of the proof virologic answer (SVO) which is defined as absence active replikatsii in blood after 24 weeks after end of antiviral therapy. Intermediate researches by virus definition in blood during antiviral therapy are used for an estimation of probability SVO and a choice of duration of treatment. Inspection during therapy includes level definition genoma a virus on 4, 12 and 24 weeks of therapy which are interpreted in comparison with level of a virus load before the treatment beginning.

Decision-making on expediency of treatment of the patient with HG should be individualised and based on:

1. Degrees of a lesion of a liver

2. Potential risks of by-effects

3. Presence of accompanying diseases

4. Probabilities of success of therapy

5. Readiness of the patient and members of his family to begin therapy.

Indications to PVT

1. Age since 3 years

2. Presence genoma a virus (DNA HBV/РНК HCV) in blood

3. The compensated lesion of a liver

4. Comprehensible hematological and biochemical indicators (Hemoglobin 12,0 g/dl for boys and 11,0 g/dl for girls, neutrophils not more low 1500 in 1 mkl, a creatinine below 1,5 mg/dl)

5. The patient and his parents have desire to be treated

6. Absence of contraindications

The individual approach at the decision on appointment PVT is necessary at sledushchih situations:

1. A noneffective course of antiviral treatment HG in the anamnesis

2. Absence or the minimum fibrosis according to morphological research

3. An acute virus hepatitis

4. A coinfection from a HIV

5. A chronic renal pathology

6. Patients after liver transplantation

7. Presence of serious accompanying diseases (disease serdechnoyososudistoj the systems which are not concerning categories, specified in contraindications; serious diseases of lungs)

8. A diabetes with ketoatsidoticheskoj a coma

9. Disturbances by coagulating system of blood (for example, at a thrombophlebitis, an embolism of a pulmonary artery) or appreciable oppression of hemopoietic function of an osteal brain;

10. Fermentopatii

Contraindications to PVT are

1. Mental diseases (including the expressed depression, suicidal thoughts or attempts, including according to the anamnesis)

2. Transplantation of solid organs (a kidney, heart, easy)

3. An autoimmune hepatitis or other autoimmune disease

4. Thyroid gland diseases if they do not give in to medicamental correction;

5. Serious diseases of heart, including the astable and uncontrollable forms existing, at least, within 6 months, previous treatment

6. Age less than 3 years

7. Hypersensitivity to preparations

8. Hemoglobinopathies (for example, a thalassemia, a sickemia);

9. Chronic renal insufficiency, a clearance of a creatinine below 50 ml/mines, necessity of carrying out of a hemodialysis

10. Appreciable disturbance of function of a liver or dekompensirovannyj a cirrhosis;

11. A cirrhosis with presence of hepatic insufficiency at (an index Chajld -

I drink> 6);

2 dispensary group

At an estimation of efficiency of therapy dynamics of biochemical indications (normalisation of transaminases), virologic research (disappearance genoma a virus (DNA HBV/РНК HCV) in blood) and a histological picture (depression nekrovospalitelnoj activity is taken into consideration at otsutsvii fibrosis increase).

Final point of antiviral treatment is achievement proof
The virologic answer (SVO) which is defined as absence active replikatsii in blood after 24 weeks after end of antiviral therapy. Intermediate researches on active replikatsii a virus in blood during PVT are used for an estimation of probability SVO and a choice of duration of treatment. Inspection during therapy includes level definition genoma a virus on 4, 12 and 24 weeks of therapy which are interpreted in comparison with level of a virus load before the treatment beginning.

Survey of the doctor is spent at least monthly, at necessity - more often. At survey it is necessary to estimate the emotional status of the patient, to tap therapy by-effects, to define interrelation of disturbance of state of health with medical product introduction. The index of mass of a body (tab is defined. 1).

** If in 12 weeks PVT depression of level of a virus load makes less than 2 log (less, than in 100 times) in comparison with initial level, or at detection in blood active replikatsii in 24 weeks of treatment, it is recommended to stop treatment.

3 dispensary group

The patients, entering into 3 group of a dispensary observation, in turn should be sectioned into the patients who have received complete course PVT and that at whom therapy has been interrupted in connection with absence of the expressed virologic answer, so-called "neotvetchiki".

The dispensary observation purpose for children who have completely graduated antiviral therapy, is the estimation of efficiency of treatment.

Presence at patient SVO is an indicator of efficiency of therapy and high probability full eliminatsii the originator from an organism.

For definition of recover after spent PVT long observation, within 5 years with frequency rate of research genoma a virus 2 times a year is necessary.

The estimation nekrovospalitelnyj prtsessov in liver tissues is spent by means of standard biochemical researches, first of all ALT.

The important criterion of an estimation of efficiency PVT is change of a histological picture. At positive dynamics at the patient infiltration of portal tracts decreases inflammatory limfogistiotsitarnaja, disappearance of zones of focal and step necrosises becomes perceptible. Further ultrozvukovaja elastografija a liver as the non-invasive method, can be applied to dynamic observation with frequency rate once a year.

Criterion of putting off from the dispensary account is only recover, that is absence active replikatsii in blood within 5 years after the termination of a course of antiviral therapy, level of transaminases within norm, stabilisation or degree reduction nekrovospalitelnyh changes in a liver, absence of advance of a fibrosis.

The dispensary observation purpose for "neotvetchikami" (patients which therapy has been interrupted in connection with absence of the expressed virologic answer) is the estimation of dynamics of development of pathological process. Such patients still have possibility of carrying out PVT at introduction of new schemes of treatment.

The following scheme of observation is recommended.

Survey of the doctor is spent 1 time in 3 months At survey it is necessary to estimate the emotional status of the patient, to tap the basic complaints, to define dynamics of clinical data - a syndrome of "small" hepatic signs (palmarnaja an erythema, vascular asterisks), colouring of an integument and visible mucosas. The sizes of a liver on Kurlovu and the size of a lien. The index of mass of a body (tab is defined. 2).

4 dispensary group

The dispensary observation purpose for children having absolute contraindications to carrying out PVT, is dynamic observation over development of pathological process, an estimation of fibrosing of a tissue of a liver, carrying out of pathogenetic therapy.

The following scheme of observation is recommended.

Survey of the doctor is spent 1 time in 6 months At survey it is necessary to estimate the emotional status of the patient, to tap the basic complaints, to define dynamics of clinical data - a syndrome of "small" hepatic signs (palmarnaja an erythema, vascular asterisks), colouring of an integument and visible mucosas. The sizes of a liver on Kurlovu and the size of a lien. The index of mass of a body (tab is defined. 3).

5 dispensary group

Tactics of observation and inspection of children who were born from mothers with anti - VGS, is defined by that the risk of realisation of a becoming infected rather is not high, maternal antibodies can circulate in blood of the child long time - till 1 year.

At the age of 6 months it is recommended to spend biochemical bloods with definition ALT, nuclear heating plant, SHCHF, GGTP, the general fiber, albumins, Y-GLOBULINS, PTI.

Repeated biochemical research and definition serologicheskih markers is spent to 12 months. At normal indicators ALT both nuclear heating plant and absence MVG in 12 months the child is considered healthy.

At allocation MVG, it is prescribed PTSR for definition genoma a virus. Only at revealing active replikatsii or repeated allocation serologicheskih markers at the age of 18 months can be diagnosed HG for the child.

After achievement by the child of 3-year-old age inspection for definition of possibility of carrying out PVT is spent.
